During the examination, fetal structure, growth, amniotic fluid volume, placental position, fetal movements, and sometimes Doppler blood flow are assessed according to clinical need. Special attention is given to systems that may change or manifest later in pregnancy, such as the brain, kidneys, digestive system, heart, and skeleton. The third trimester scan does not replace previous scans, but rather provides an additional layer of security - an examination that allows a renewed and precise look at the fetus at a stage when some findings may be clearer.

Who is third trimester screening suitable for?

The examination is particularly recommended in cases of previous findings in pregnancy, need for growth monitoring, suspected growth restriction, polyhydramnios or oligohydramnios, multiple pregnancy, or when there is a need for focused assessment of organs that develop and change even in the third trimester - such as the brain, heart, kidneys and digestive system.

The third trimester scan is not mandatory for all pregnant women, but is highly recommended for women with risk factors. However, many women choose to perform it even without special risk factors, to get a complete picture of the fetus's condition before delivery and to alleviate anxieties. IUGR (Intrauterine Growth Restriction) – intrauterine growth restriction – is a condition where the fetus is not growing at the expected rate for the gestational week. The causes can be poor placental function, maternal diseases, genetic factors and more. Treatment depends on the severity of the condition and gestational age: increased monitoring with frequent Doppler tests, rest, and in severe cases – planned early delivery. Early diagnosis through the third trimester scan enables rapid response and preservation of fetal health.
Doppler examination measures the speed and direction of blood flow in specific blood vessels. In the third trimester scan, we mainly check: the umbilical artery – reflecting placental resistance; the middle cerebral artery (MCA) – indicating the fetal condition and its ability to maintain blood flow to the brain; and sometimes liver veins as well. Abnormal Doppler values can precede changes in heart rate and assist in making early treatment decisions.
The third trimester scan is usually performed between week 28 and week 34 of pregnancy. The preferred timing is around weeks 30–32, as at this stage reliable measurements of fetal growth can be obtained and there is still time to act if necessary. In women with risk factors, additional follow-up examinations may be performed at higher frequency, according to the doctor's recommendation.
